    Summary table assessment of the prevalence of 12 procedure codes in the Mini-Sentinel Distributed Database using Healthcare Common Procedure Coding System (HCPCS) codes, specifically "Injection busulfan 1 MG" (HCPCS code J0594), "Busulfan; oral 2 MG" (HCPCS code J8510), "Cyclophosphamide oral 25 MG" (HCPCS code J8530), "Methotrexate oral 2.5 MG" (HCPCS code J8610), "Cyclophosphamide lyophilized 100 MG" (HCPCS code J9093), "Cyclophosphamide lyophilized 200 MG" (HCPCS code J9094), "Cyclophosphamide lyophilized 500 MG" (HCPCS code J9095), "Cyclophosphamide lyophilized 1 G" (HCPCS code J9096), "Cyclophosphamide lyophilized 2 G" (HCPCS code J9097), "Injection trastuzumab 10 MG" (HCPCS code J9355), "Finasteride 5 MG" (HCPCS code S0138), and "Chlorambucil oral 2 MG" (HCPCS code S0172). Results were generated using the Mini-Sentinel Distributed Query Tool and the HCPCS Summary Table. Summary tables are precompiled stratified counts. Queries were run in the outpatient setting. Queries were executed in November 2011. This report includes data from 13 Data Partners.
